Three piece construction, in bronze gilt with azure blue and black enamels, obverse illustrating a Crutch Cross with crossed swords piercing the centrepiece, the four arms in black, with a central raised disk in blue bearing a raised Tryzub, reverse inscribed in Ukrainian "C Y B" (SUV) on the arm at 12 o'clock and number impressed "02198" on the arm at 6 o'clock, measuring 39 mm (w) x 39 mm (h),
